Michigan Department of Health and Human Services, Division of Vital Records (Michigan Vital Records) issues certified copies of Michigan birth certificates, Michigan death certificates, Michigan marriage certificates and Michigan divorce certificates for events that occurred within the State of Michigan. Affidavit of Parentage records filed in the Central Paternity Registry since June 1, 1997, are also available online. You may order copies of Michigan Vital Records through VitalChek on an expedited basis. Michigan Vital Records charges a $12.00 RUSH fee for each online order, in addition to VitalChek's $11.50 credit card handling charge.

A certified death certificate that contains an apostille. This authenticated record is printed on special safety paper and contains a raised, embossed seal. It also contains the Great Seal of Michigan, as required by some foreign countries for adoption, marriage, establishment of citizenship, work visa, or court action in a foreign country. An authenticated record (apostille) is not required for foreign travel. When ordering an apostille, you must provide the name of the FOREIGN country where the document will be used. An apostille CANNOT be applied to a document that will be used in the United States. Please allow an additional 2 -3 weeks processing time at the Office of the Great Seal, in addition to the regular processing time.
Marriage Certificate
A certified copy of the marriage certificate. This is not a copy of the marriage license. Marriage records are available for events that occurred within the State of Michigan from 1867 to present. Marriage certificates are not available until at least 30 days after the event occurred.

If you need a copy of the divorce decree with all of the particulars of the divorce, it is a court document and must be requested from the court that finalized the divorce. Divorce records are available for events that occurred within the State of Michigan from 1897 to present. Divorce certificates are not available until at least 30 days after the event occurred.

A certified birth certificate that can be used for genealogical purposes. Important Note - To be eligible for this record, the event date must be 100 years old or older.
Affidavit of Parentage
An Affidavit of Parentage is a legal document stating that the father has accepted all legal responsibility for the child. Affidavits are available for filings that occurred from 6/01/1997 to the present that are available within the Central Paternity Registry. To request an Affidavit of Parentage, please select 'Birth Certificate' under 'Start Your Order' above, to begin the ordering process. You will need to select 'Affidavit of Parentage' as the 'Reason' for request.
If you would like an Affidavit of Parentage prior to 6/01/1997, please contact the Probate Court in the county where the mother resided at the time of signing or the Probate Court in the county where the child was born.
Who Can Order?
Affidavit of Parentage records are restricted documents and are only available to the mother, father or child named on the record. Additional eligibility options are available by contacting the State Vital Records office directly.
Heirloom Birth Certificate
This is a souvenir document that can be passed from generation to generation as a cherished keepsake. The souvenir heirloom birth certificate is 8.5 x 11 inches (suitable for framing). There are five different designs to choose from, created from original artwork. These beautiful keepsake certificates are printed in color and can be purchased as a remembrance of that special occasion. The sale of each heirloom birth certificate will provide $20.00 to the Children’s Trust Fund of Michigan.
Non-certified heirloom birth certificates can be used for souvenir purposes only and cannot be used for proof of citizenship or any other official or legal purpose.
